Forms are containers that hold Items and define how those items should be displayed. Forms are used to construct tests and can contain items from multiple banks. See Authoring a New Form for more information.
- Go to Authoring > Forms.
- On the QTI Forms page, you can search for forms and perform the following actions:
- Edit the form.
- Download Test Map – Download a CSV file containing the form name, items, points, key, standards, etc.

Upload Paper Form – Upload a printable test form in PDF format. After uploading a form, the following options become available in the Actions dropdown:
- Print the PDF. The PDF can also be printed from the Administration card (see Admin Card) and from the Proctor Dashboard. Administrations may include multiple forms with PDFs.
- Replace the PDF.
- Delete the PDF.
- Clone the form.

Note: TestNav forms have only two actions: Preview Test Map and Upload Paper Forms, which can then be printed.
